Left end is the 25th percentile, right end is the 75th, the tick is the median. No supplier triangle yet: yours is drawn when you run your NPI. The 90th percentile target is locked on the public index.
Three real supply codes from the public record. Not your book until you bring yours.
Since 2022, every insurer must publish what it pays for every supply code.45 CFR 147.212
Same supply. Same code.Different checks.
One knee orthosis code, L1832, is contracted from $313 to $1,121 in the California commercial market. Score your supplier NPI, free.

Where this applies, and where it does not
Medicare DMEPOS is not negotiable. Your commercial contracts are.
Medicare DMEPOS fee schedules are set by CMS and by competitive bidding, so they are not individually negotiable, and nothing here pretends otherwise. The commercial side is a different market. Since 2022 every insurer has had to publish its contracted rates in machine readable federal filings, which is what makes an independent benchmark on your commercial book possible at all.
The thing nobody could see before
One code does not have one price.
E0601 is a CPAP device. Below is the band the commercial market published against that one code, read from the same federal filings on the same day, in three states. The code is identical in all three. The band is not.
| State | 25th | Median | 75th | 75th / 25th | Rate records |
|---|---|---|---|---|---|
| California E0601 | $44 | $314 | $1,217 | 27.7× | 219,409 |
| Texas E0601 | $56 | $252 | $336 | 6.0× | 64,356 |
| Florida E0601 | $54 | $301 | $401 | 7.4× | 30,169 |
Read from /api/rate-benchmark on 14 Sep 2026, commercial market, statewide basis, never a national average. Rental against purchase, modifiers and place of service all sit inside a single code, so a band is a starting position and not a verdict on any one line of yours. Reading your lines against it is what the working session is for.

A record beats an opinion, and it has to exist before the letter lands.

A sample of the DME, HME and orthotic codes the index holds, not the whole set, and not a claim about your state. Rate records is the count of indexed rate lines behind that band. The 90th percentile target is locked on the public index and unlocks inside the product. Bring your top 20 codes and we read yours on the call.
Consultancies charge $75K to $350K to do this. You start at zero.
Every price we charge is published on one page. A score on a single NPI takes about two seconds, with no account, no card and no sales call, which is not true anywhere else in this field. Nothing has existed between a free lookup and a six figure engagement. That gap is the whole business.
